WAIVER OF LIABILITY, TERMS AND AGREEMENTSWAIVER OF LIABILITY, TERMS AND AGREEMENTS
I (also referred to as “client”) have requested that a Golden Beam of Light, LLC and www.Rosine Kushnick.com’s practitioner (Rosine Schiess-Kushnick) (together also be referred to as “GBL”) provide me with soul readings of any type, Akashic record readings, energy balancing sessions, integration sessions, as well as classes, workshops, retreats, or other consultative services (together referred to as “Services”).
1. Golden Beam of Light, LLC and www.Rosine Kushnick.com practitioners or authorized agents (and the Services they provide) do not diagnose conditions nor do they prescribe or perform medical treatment, prescribe substances, nor interfere with the treatment of a licensed medical professional. Their advice and treatments/sessions are not a substitute for conventional medical diagnosis or treatment for any medical or psychological condition and do not take the place of medical care. I understand that it is recommended that I see a licensed physician or licensed health care professional for any physical or psychological ailment that I may have. Their advice, treatments, sessions and other Services can complement any medical or psychological care.
2. Golden Beam of Light, LLC and www.Rosine Kushnick.com makes no guarantees, projections or estimates as to how well I will do or the level of success I might achieve by using the information and strategies covered in your consultations or sessions or any Services provided by GBL and www.Rosine Kushnick.com providers or authorized agents.
3. I understand that any information, communication and or consultations with GBL, or Rosine Kushnick are for educational, spiritual and entertainment purposes only. No reading given by GBL or Rosine Kushnick are intended to nor should ever take the place of professional services including but not limited to: medical, legal, financial, business and or psychological. I understand that GBL, Golden Beam of Light, LLC and www.Rosine Kushnick.com’s practitioner (Rosine Schiess-Kushnick) accept no liability and or responsibility for any actions and or decisions that I choose to take or make based on my consultation with Rosine Kushnick or GBL.
4. In the event of a private session, I understand that the appointment will be cancelled if not rescheduled prior to 24 hours of the appointment time. I understand that if I arrive or call in late, the session will still end at the designated time. I understand that this is a subscription and I can opt out at any time by cancelling my subscription. Each payment will give me access to one month worth of calls.
5. I understand that all information shared in a session or Services will be held in confidence and that GBL has the right to terminate a call or a program at any time if my comments and/or behavior are deemed by GBL to be inappropriate, abusive, or threatening.
6. Except in the case of gross negligence or malpractice, I, and my heirs or my representative(s) agree to fully release and hold harmless GBL, Rosine Kushnick, www.Rosine Kushnick.com and/or Golden Beam of Light, LLC from and against any and all claims or liability of whatsoever kind or nature arising out of or in connection with my sessions or participation in any Matrix Energetics or consultative sessions, Soul Readings, any classes, workshops, retreats or any other Services that they may provide.
7. I shall defend, indemnify, and hold harmless Rosine Kushnick, Golden Beam of Light LLC, and her employees, directors, independent contractors, related entities, trustees, affiliates, and successors from and against any and all liabilities and expense whatsoever – including without limitation, claims, damages, judgments, awards, settlements, investigations, costs, attorney’s fees, and disbursements – which any of them may incur or become obligated to pay arising out of or resulting from advice given during this coaching contract, excluding, however, any such expenses and liabilities which may result from a breach of this Agreement or sole negligence or willful misconduct by Golden Beam of Light, or any of her affiliates or successors. I shall defend Golden Beam of Light in any legal actions, regulatory actions, or the like arising from or related to this Agreement.
8. I am ready to dive deep into the transformational work with Rosine; reconfiguring how I show up in my love relationship, both vibrationally and behaviorally, so that I can create lasting, secure, loving and uplifting relationships, both with my intimate partner, and also with friends and family.
9. I commit to showing up fully and consistently. I understand that Rosine is equally committed to showing up and holding a sacred space for me to move deeper into wholeness.
10. I take 100% responsibility for my success in this program and all sessions included.
11. I understand that Rosine is not a licensed therapist and that I am responsible for all my thoughts, feelings and action.
12. I understand that payments are non-refundable.
13. I understand that Rosine reserves the right to terminate this agreement at any point for any reason.
14. I hereby grant permission to Golden Beam of Light, LLC (hereinafter “GBL”) and www.RosineKushnick.com (“RK”), the rights of my image, in video or still, and of the likeness and sound of my voice as recorded on audio or video.
15. I hereby release GBL and RK, its affiliates, employees, representatives and agents from any and all claims, demands, costs and liability that may arise from the use of these images and recordings.
16. I understand that Golden Beam of Light, LLC and www.RosineKushnick.com are aware of the private nature of the videos and I understand that my images and recordings are for the sole purpose of supporting and inspiring other clients and participants. I waive the right to inspect or approve the finished product wherein my likeness appears.
17. This consent is given in perpetuity and does not require prior approval by me.
18. Golden Beam of Light LLC, agrees to keep information you share confidential.
19. By purchasing this program and or session, I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby.
20. I acknowledge that I have read this waiver of liability form. I fully understand its terms and conditions, and further understand that I am waiving and giving up my right to sue GBL, Golden Beam of Light, LLC and www.Rosine Kushnick.com or their practitioners or authorized agents. I acknowledge that I am signing this agreement voluntarily, and intend by my signature for this to be a complete and unconditional release of liability to the greatest extent allowable by law.
